Manhattan, New

performing Breast Augmentation with Silicone Breast Implants
for more than 15 years. The NYC plastic surgeon has
been a participant in the FDA Adjunct Study, which permitted
silicone breast augmentation under certain, limited
circumstances. Since late 2006, when the FDA gave its

individual basis. Each form of breast implant offers
its own advantages and disadvantages. In general,
patients who do well with saline breast implants and those
who have adequate body fat and adequate breast tissue are
not seeking a very large breast augmentation.
If a patient however has relatively little breast tissue
(small to medium A cup) and is very lean, she may have a
superior result with silicone breast implants, especially if
she is planning to achieve a more significant increase in
size. In cases where the size of the breast implant is
relatively large compared with the patient's breast volume,
silicone breast implants offers the distinct advantage of
having a softer, more natural feel as well as less change of
showing ripples.
While silicone
breast implants offer the advantage of a potentially softer
and more natural feel, there are two relative disadvantages
to take into account. Because the come prefilled, they
require a somewhat longer scar. While a scar under the
breast is always an option, and a scar on the border of the
areolus is possible when the areolus is large enough,
many patients prefer to have no scar on the breast.

The Breast Augmentation NYC Plastic
Surgeon offers breast augmentation with saline-filled
implants. This has been demonstrated to be a safe operation with
very high satisfaction rates, both short and long term.
The breast augmentation / breast implants plastic surgery operation involves several important choices on the part
of the patient. These include implant size, implant position
(on top of or under the pectoralis muscle) and the choice of
location for the small surgical access scar. Implant size is
selected based on a detailed consultation, aided by viewing
many examples of before and after results, as well as
obtaining a full understanding of the patient’s goals and
concerns. Implant size will also depend on the patient’s
individual anatomic considerations including skin laxity and
chest diameter.
With regard to implant position, the breast augmentation NYC plastic surgeon generally
recommends positioning the implants under the pectoralis
muscle. This is for three reasons: The muscle covering the
top part of the implant results in a more natural appearing
breast with less likelihood of a visible implant edge or
visible rippling. Secondly, there is a reduced likelihood of
capsular contracture, or hardness, developing over time.
This is believed to be due to the massaging action of the
muscle on the scar tissue surrounding the implant. Finally,
the submuscular implant position allows for a better view of
the breast when a mammogram is obtained.
The scar for breast augmentation can be placed either in the
underarm, under the breast itself, or at the lower border of
the areolus (the pigmented skin surrounding the nipple).
Many patients, when given the option, prefer an underarm
scar because it removes all scarring, and anxiety relating
to scarring, from the breast. Dr. Rapaport performs this
procedure using an endoscope. This means that instead of
bluntly tearing the lower border of the muscle, and risking
implant malposition due to incomplete muscle division, the
muscle is precisely divided under direct visualization,
allowing for precise implant positioning and control of any

Saline Breast Implants vs Silicone Breast Implants
Now that the FDA has authorized the use of silicone gel
breast implants for breast augmentation on all women (age 22
and over), many patients are again asking which type of
breast implant is best for them.
Board Certified Plastic Surgeon, David P. Rapaport, MD has
over 15 years of experience working with both silicone
breast implants and saline breast implants. The doctor
understands that many potential patients will respond to the
approval of silicone breast implants by believing that
silicone must be a better product -- both because it is
"new", and its arrival has been anticipated and praised by
nearly all plastic surgeons, including Doctor Rapaport.
In fact, in the Board Certified Plastic Surgeon's view, the
issue is more complicated and as he explains, choosing the
type of breast implant always involved trade-offs.
These issues are discussed with each patient after she is
examined in the doctor's Manhattan, New York surgical

saline breast implants for most of his patients.
The exceptions are those who are extremely thin, and/or
those patients desiring very large breast augmentations
(over 400 ccs).

longer scar for insertion, this scar usually being under the
breast. Because saline implants arrive empty, and are
filled once they are in the body, they can be rolled up and

implants appear more natural: in the vast majority of

breast implants are probably more likely than saline breast
implants to form capsular contractures, which lead to a hard
and firm, unnatural feeling breast. Finally, many of
